An optometry practice equipment list is a business plan, not a shopping cart
Writing an optometry practice equipment list is one of the first — and most consequential — decisions in opening a new office. Every line on it is a fixed cost that arrives before your first full schedule does, and the order in which you buy determines how much runway you have left when patient volume finally shows up. Approached as a shopping cart, the list becomes a wish list. Approached as a business plan, it becomes a sequence: what has to be in the room on opening day, what can wait for the second quarter, and what only earns its place once a specific service line exists to justify it.
This checklist is organised the way a practice actually gets built — by phase, and by the workflow each instrument sits inside. It is written for practice owners and clinic buyers, not for patients, and it deliberately spends as much time on service, parts and warranty as it does on specifications, because those are the variables that decide what a piece of equipment costs you over eight years rather than on the day it ships.

Buy in phases: what opening day actually requires
The most common cold-start mistake is equipping for the practice you expect in year three. The published practice-management advice is consistent on this point: as Eyes On Eyecare's cold-start guide puts it, "You should only equip one exam lane to start and can add equipment to the second lane when patient volume increases." One complete, well-specified lane will out-perform two half-finished ones, and it leaves capital available for the marketing that fills them.
Three questions separate a Phase 1 purchase from a Phase 2 purchase:
- Can you see a patient without it? If the answer is no, it is Phase 1. A lane without a phoropter is not a lane.
- Does it shorten the exam or free the doctor? Pretest instruments — autorefractor/keratometer, lensmeter, non-contact tonometer — pay for themselves in doctor-minutes long before they pay for themselves in billing. They are Phase 1 in any practice that plans to run a technician.
- Does a service line already exist to justify it? Imaging, perimetry and specialty equipment follow the patients, not the other way round. They are Phase 2, and buying them early converts working capital into depreciation.

Phase 1 — the core exam lane
This is the non-negotiable set. Configure it once, well, and repeat it lane-for-lane as you grow — a second lane that matches the first is faster for staff to work in and cheaper to keep in parts.
Refraction: the autorefractor/keratometer, phoropter and acuity system
Objective refraction is where the exam starts and where the most time is saved. An autorefractor/keratometer (ARK) gives the technician a starting point and, in the same pass, corneal curvature readings that matter for contact-lens work. From there the refraction is refined at the phoropter. The manual-versus-digital decision is the single biggest ergonomic choice in the lane: a digital refractor moves the doctor out of the reach-and-crank posture, stores the sequence, and pairs with the acuity chart, while a manual head has fewer failure modes and no software dependency. Plan the room around the refraction distance before you plan it around the furniture. A mirrored lane folds the optical path, and the geometry — not the floor plan you sketched — sets the minimum room length.
Slit lamp and eye-health examination
The slit lamp is the instrument you will use on every patient, every day, for fifteen years, which makes optical quality and mechanical feel worth more here than on almost anything else on the list. If you are equipping for outreach, nursing-home rounds or a paediatric-heavy schedule, a portable slit lamp is a genuine second instrument rather than a compromise — but it is not a substitute for a tabletop unit in a fixed lane.

Tonometry
Intraocular pressure measurement can be handled at the slit lamp with an applanation attachment, or on a standalone non-contact tonometer (NCT) in pretest. The NCT's argument is workflow, not accuracy: it moves the measurement to a technician, needs no anaesthetic drop, and removes a step from the doctor's sequence. The chair, stand and instrument table
Furniture is the line most often value-engineered and most often regretted. The chair and stand are load-bearing for accessibility compliance, for the doctor's back, and for whether the lane can be reconfigured in five years without replacing everything. Motorised height, a counterbalanced instrument arm, and upholstery you can still re-order a decade from now are not luxuries — they are what makes the unit serviceable. One practical note: buy the chair, stand and instrument table as a system if you can. Mismatched heights between a stand-mounted phoropter and a separately-bought table are a daily annoyance that no specification sheet warns you about.
Phase 1 — pretest and the optical dispensary
The dispensary is where a large share of practice revenue is actually captured, and it is chronically under-equipped in first-year plans. Two of its instruments belong in Phase 1.
Lensmeter
Reading the spectacles a patient walks in wearing is the cheapest clinical information you will get all day. As the cold-start guidance puts it plainly, "All new practices will need to invest in a lensometer." PD, centration and dispensing measurement
Centration errors are the quiet cause of remakes. Edging: in-house lab or outsource?
This is the biggest Phase 1 fork on the list, and it is a business decision rather than an equipment one. An in-house lens edger gives you same-day single-vision jobs, control over remakes, and margin you currently hand to a lab; it also gives you a machine to maintain, consumables to stock, and a staff member who has to be trained on it. A reasonable default for a cold start: outsource edging in year one, and revisit the moment your monthly job count makes the maths obvious.
Phase 2 — imaging and the instruments that follow patient volume
These are the purchases that reward patience. Each one should be tied to a service line you can already see in your schedule.
Retinal imaging
A non-mydriatic retinal camera is usually the first Phase 2 instrument a practice adds, because it changes the patient conversation as much as the record: an image on screen is the most persuasive thing in the room. Binocular indirect ophthalmoscopy
A binocular indirect ophthalmoscope and a condensing lens remain the standard for peripheral retinal examination, and no camera replaces them. Portable and handheld instruments
If your growth plan includes a satellite location, school or care-home screening, or a paediatric panel, the portable equipment range is a different Phase 2 route from imaging: handheld autorefractors, portable slit lamps and portable cameras let you take the exam to the patient. The non-instrument essentials
Every equipment list under-counts this category, and it is where opening-week surprises live.
- Consumables and spares: bulbs for every illuminated instrument, tonometer prisms and covers, chin-rest tissue, cleaning supplies, fuses. Keep a spare bulb for each lamp on site, not on order.
- Frame board, dispensing table and lab supplies: the dispensary needs its own fit-out, including a frame warmer and basic adjustment tools, even if you outsource edging. See lab supplies for the consumable side.
- Practice management and EHR: confirm before purchase which instruments your software will actually accept data from. Instrument-to-EHR connectivity is a specification, and it is one that vendors describe loosely.
- Trial lens set, retinoscope, transilluminator, BIO headset and condensing lenses: individually small, collectively a meaningful line.
- Furniture, waiting-area seating and signage: not on any instrument list, always on the opening-day invoice.
- Calibration and installation: ask who performs it, whether it is included, and what recertification will look like in year two.

New, open-box or financed: making the 2026 budget work
There are three levers on a first-year equipment budget, and most practices pull all three.
Open-box and end-of-line units. Demonstration stock, returned units and end-of-line items carry the same support as new stock in most cases and are the least-risky way to compress a budget — provided the warranty and parts position is written into the quote rather than assumed. Financing. Spreading the cost of a lane over its useful life rather than paying for it out of opening capital is ordinary practice, and financing options are available on our equipment.
First-year expensing. Section 179 of the US tax code lets a business elect to expense qualifying equipment in the year it is placed in service rather than depreciating it over several years, and it is the reason so much equipment is ordered in the fourth quarter. As published by Section179.org, which attributes the figures to Rev. Proc. 2025-32, the 2026 maximum deduction is 2.56 million, with the phase-out beginning once qualifying purchases exceed 4.09 million and completing at 6.65 million; the same source reports a 100% bonus depreciation rate for qualified property acquired and placed in service after 19 January 2025, applied after any Section 179 election. Those are the figures that source publishes and attributes to the revenue procedure — we have not read the revenue procedure itself, and the thresholds are inflation-adjusted annually.

The question most equipment lists leave out: who services it
An equipment list is a list of relationships as much as a list of machines. The instrument that is down for three weeks in February is more expensive than the one that cost more in January, and nothing on a specification sheet tells you which one you are buying.
Four questions belong on every quote, whatever you are buying and whoever you are buying it from:
- Who performs the service, and where are they? A domestic service department and a shipping label to another country are different products.
- Are spare parts stocked, or ordered on demand? Availability, not price, is what determines downtime.
- What exactly does the warranty exclude? Consumables, lamps and shipping are the usual carve-outs. Ask in writing.
- What does support look like after the sale? Named contact or a queue.

Is open-box or pre-owned equipment a reasonable choice for a cold start?
Yes, if the warranty, calibration status and parts availability are established in writing before purchase. What should be on an equipment quote besides the instrument?
