What counts as "used" — four labels doing very different jobs
The pre-owned market runs on four words, and only one of them has a fixed meaning:
- Used / pre-owned. The unit was in service somewhere. Nothing is implied about what was done to it since. This is the honest label, and it is the one most marketplace listings should carry.
- Refurbished. Work was performed. Which work is entirely up to the seller, and the sellers who use the word rarely define it on the page — which is why the rest of this guide is mostly about pinning it down.
- Demo / demonstration unit. The instrument was used by the manufacturer or the equipment provider for trade shows, training, or in-office demonstrations. Low hours, usually current-generation, often never sold to an end user.
- Open-box. The unit was sold or shipped and came back, or the carton was opened and the instrument never entered clinical service. It is functionally a new instrument that has lost its "new" label — and it is the grade most buyers never think to ask for.
Those four labels frequently sit in the same search results and the same conversation. Getting a seller to say which one applies, in writing, is the first useful thing you can do.
"Refurbished" is a marketing word, not a specification
Read the used-equipment page of an established seller in this market and a pattern appears quickly. On one such page, checked while writing this, the language is confident and the content is thin: equipment is "inspected, serviced, and reviewed" before sale, and a section headed "Refurbished Equipment Guaranteed to Perform" is followed by no warranty terms at all. What that page does not publish is the part a buyer can act on — which components are replaced as a matter of course, what the verification procedure is, what the warranty covers, and how long it runs.
That is not necessarily bad faith. Refurbishment genuinely varies unit by unit, and a seller who publishes a fixed process has to honour it. But it does mean the burden falls on you. A useful refurbishment description answers four questions in specifics, not adjectives:
- What was replaced? Named consumables and wear items — illumination lamps or LED modules, batteries, drive belts, filters, chinrest paper holders, cables, mains fuses. "Replaced as needed" is not an answer.
- What was verified, and against what? Was the instrument checked against the manufacturer's stated procedure and tolerances, or against the technician's judgement that it "reads fine"?
- What was not done? The most informative sentence in any refurbishment write-up. Optics not disassembled, printer mechanism not serviced, touchscreen digitiser original — all perfectly acceptable disclosures, and all far better than silence.
- Who did the work? A factory-trained engineer, a general medical-equipment technician, or the seller's shipping department.
If a seller cannot answer those four, you are not buying a refurbished instrument. You are buying a used one with a nicer adjective, and you should price and warranty it accordingly.
Parts availability decides more used purchases than condition does
Here is the failure mode that catches practices out. The instrument arrives, works beautifully for eleven months, and then a single component fails — a lamp housing, a keypad, a drive assembly. The unit is not broken in any deep sense. It simply cannot be returned to service, because nobody in the country stocks that part for that model any more.
Condition is visible in photographs. Parts availability is not, and it is the harder constraint. Before you buy any pre-owned instrument, establish three things about its model, independent of the individual unit:
- Is the model still supported? A generation-old instrument from a line that is still in production is a very different proposition from the last survivor of a discontinued line.
- Does anyone in the US hold parts for it? Not "can they be ordered" — held, on a shelf, in this country. An overseas parts order on a down instrument is measured in weeks.
- Are the consumables generic or proprietary? Proprietary consumables are a permanent tax on a cheap acquisition, and they end the moment the manufacturer stops making them.
This is the one place where buying pre-owned from an equipment provider and buying from a marketplace genuinely diverge, and it is checkable rather than a matter of trust: look at whether the seller publishes a parts catalogue for the brand at all. The general rule is unglamorous and reliable: buy pre-owned in the brands whose parts you can already see on a shelf. A house brand supported by the company selling it — with, in our case, a technical service department of factory-trained engineers and a 3,000-square-foot warehouse holding "a stock of supplies, spare parts, and a wide variety of inventory" — is a materially different risk from an orphaned model bought from an anonymous account.
Calibration and verification: what to ask for in writing
Every measuring instrument in the lane drifts, and a period out of service on a pallet is exactly when drift goes unnoticed. Whatever the seller says about condition, ask for the verification record — and if there isn't one, plan to establish a baseline yourself in the first week, before the unit starts producing numbers you rely on.
What to establish, by instrument type:
- Autorefractors and autorefractor/keratometers. Check against the manufacturer's model eye and the supplied test target, on both the refraction and keratometry sides. Compare a handful of readings against your existing instrument on real eyes before you retire the old one — a systematic offset shows up immediately in that comparison and never shows up in a spec sheet. - Lensmeters. Verify against known reference lenses across sphere, cylinder, axis and prism, and check the PD and progressive-lens modes specifically — they are the functions most likely to have been ignored by a previous owner. See the auto-lensmeter overview for what a current unit should do.
- Tonometers. Follow the manufacturer's own verification procedure at the interval the manual specifies, and get the last verification date from the seller in writing. This is the instrument where an unverified used purchase is least defensible.
- Slit lamps and biomicroscopes. Mostly mechanical and optical: check illumination through the full range, filter wheel detents, magnification changer, joystick and vertical drive travel, and look for fungus or delamination in the optics under a bright field.

Which instruments buy well used — and which deserve extra scrutiny
Generally sound pre-owned buys
- Exam chairs and stands. Electromechanical, long-lived, and easy to assess in person. Upholstery is cosmetic and replaceable; the motor, the lift, and the recline mechanism are what to test. See the chair and stand guide, and the current refraction units for comparison.
- Instrument tables. Almost pure mechanics. Check the lift under load and the tabletop for warping.
- Manual phoropters and trial lens sets. Few electronics, decades of service life. Inspect for chipped or hazed lenses and sticky dial detents.
- Slit lamps of a supported generation. The optics either pass inspection or they don't, and the verdict is available in five minutes with the instrument in front of you.
Buy with extra scrutiny
- Anything with a proprietary consumable — printer rolls, single-source probe covers, sealed cartridges. Confirm the consumable is still manufactured before, not after.
- Software-dependent imaging instruments. The hardware may be perfect while the software will not install on a current operating system, the licence is locked to the previous owner, or the export format no longer imports into your records system. Ask specifically about licence transfer.
- Battery-powered handhelds. Battery packs age on the shelf whether or not the instrument is used, and on older models they are often the first part to become unavailable. Compare against the current portable equipment line before assuming used is the economical route.
- Instruments with a printer. The print mechanism, not the optics, is usually the first thing to fail on a heavily used unit — and it is the part sellers least often mention.
Open-box: the grade most buyers never ask for
If your reason for shopping pre-owned is budget rather than a specific vintage instrument, open-box is usually the better trade and it is systematically under-searched. An open-box unit is current-generation, carries the same parts and service path as a new one, and has none of the unknowns this guide has spent five sections describing — no unrecorded service history, no orphaned model risk, no ambiguity about what "refurbished" meant.

Ten questions to put on any used-equipment quote
Ask these in writing, and keep the reply. A seller who answers all ten plainly is a seller worth buying from, whatever the grade of the unit.
- What is the exact model and serial number, and may I have photographs of the serial plate?
- How old is the unit, and is that from documentation or an estimate?
- Where did it come from — a practice, a trade-in, an auction lot, or a liquidation?
- What specific work was performed, by whom, and on what date?
- What was checked or verified, against what reference?
- What is the warranty term, what does it cover, and what voids it?
- If it fails in month two, who pays for the repair, the parts, and the freight?
- Are spare parts for this model currently held in the US?
- Are the consumables it needs still manufactured?
- Does it ship with the original manual, power supply, model eye or test target, and any software licence — transferable to me?
Question 7 is the one that separates sellers most reliably. Freight on a lane instrument is not trivial, and "we'll take care of you" is not a warranty term.
First-year expensing applies to used equipment too
One point that shifts the arithmetic and that a surprising number of buyers do not realise: for Section 179 purposes, equipment does not have to be new. Section179.org states plainly that "New & Used Equipment: Qualifies for full Section 179 deduction," and that "New or used equipment qualifies." For tax years beginning in 2026 the same source gives a deduction limit of 2,560,000, phasing out dollar-for-dollar once qualifying property placed in service exceeds 4,090,000, with bonus depreciation at 100% applying after Section 179.
Those figures come from that published summary, not from the underlying revenue procedure, which we did not read — the distinction matters if you are relying on the number rather than orienting by it. The general shape is the useful part: a well-chosen pre-owned instrument placed in service this year is handled the same way a new one is, which narrows the gap between a cheap used unit and a supported open-box unit considerably.

Red flags in a used-equipment listing
- Stock photography. If the seller will not photograph the actual unit, including the serial plate and any cosmetic damage, there is a reason.
- "Powers on." A statement about electricity, not about function. It usually means nothing further was checked.
- "Sold as-is, no returns" on a measuring instrument. Acceptable on a stand. Not on anything that produces a number you act on.
- No serial number in the listing. Without it you cannot ask the manufacturer whether the model is still supported.
- A seller with no service capability. If the answer to "who fixes this if it fails" is "the manufacturer, I suppose," you are the service department now.
- Anonymous accounts and auction lots. Ophthalmic instruments are heavy, delicate and awkward to ship. Auction freight damage on an uninsured lot is a total loss with no recourse.
