A corneal topographer is a foundational diagnostic instrument for any practice that fits contact lenses, screens for keratoconus and other ectatic disease, evaluates irregular astigmatism, or plans cataract and refractive surgery. Most systems work on the Placido-disc principle: concentric illuminated rings are projected onto the tear film, and the reflected pattern is analyzed to reconstruct the shape and curvature of the anterior cornea. The software renders that data as color-coded maps — steeper zones in warm colors, flatter zones in cool colors — so you can see surface irregularities at a glance and track how they change over time.

How to choose a corneal topographer

  • Measurement scope: Placido-disc topographers characterize the anterior corneal surface — curvature, astigmatism and keratoconus screening — which covers the needs of most contact lens and general practices. If you also need posterior-surface elevation and full-thickness pachymetry, that is the domain of Scheimpflug tomography, a distinct and typically higher-cost class of instrument.
  • Ring coverage and resolution: The number of Placido rings (or mires) and the corneal area sampled affect how much of the cornea, including the periphery, you capture and how finely surface detail is resolved. Some platforms offer a choice here — the Tomey TMS-4N, for example, is available in 25-ring or 31-ring cone configurations.
  • Analysis maps and screening software: Look at the map set and indices included: axial, tangential, refractive, Gaussian and elevation maps, Fourier or Zernike analysis, dedicated keratoconus screening, and differential or comparison maps that quantify change between visits. Progression tracking is essential for monitoring keratoconus and post-surgical corneas.
  • Stand-alone versus multifunction: Decide whether you want a dedicated topographer (Huvitz HTG-1, Ezer Onyx, Tomey TMS-4N) or an all-in-one unit that consolidates several exams. What is the difference between a corneal topographer and a corneal tomographer? A topographer maps the shape and curvature of the anterior corneal surface, typically using Placido-disc reflection, which is ideal for contact lens fitting, astigmatism evaluation and keratoconus screening. A tomographer — usually Scheimpflug-based — reconstructs the cornea in three dimensions, adding posterior-surface elevation and pachymetry data. Tomography provides a more complete picture but is a costlier, separate class of device; most practices are well served by a quality Placido topographer.

Which topographer is best for keratoconus screening? Any model with dedicated keratoconus indices and differential or comparison maps is well suited, because tracking change over time is central to detecting and monitoring ectasia. Can a corneal topographer help with contact lens fitting? Yes. What is dry eye or meibography analysis, and which models include it? Beyond corneal mapping, some topographers add tear-film and meibomian gland imaging to help evaluate dry eye disease — assessing tear meniscus, break-up and gland structure.
